Who is a Candidate?

❖ FTM (female to male) top surgery is performed to remove the breast
tissue (mastectomy) from both breasts and create a masculine
appearance to the chest. There are many techniques used to perform
FTM top surgery. This will be decided upon after your consultation
with Dr. Chao.
❖ MTF (male to female) top surgery is performed to enhance the size and
shape of the breasts. This is done to create a more feminine
appearance to the chest. There are many techniques used to perform
top surgery. This will be decided upon after your consultation with Dr.
Chao.
❖ Both FTM and MTF patients should meet the following criteria:
➢ You have been diagnosed with persistent gender dysphoria.
➢ You have been under the care of a mental health professional
who can provide a letter of recommendation supporting your
decision to undergo surgery.
➢ You have been living as a member of your desired sex.
➢ You are of the age to make consent.
➢ You have a positive outlook and realistic expectations.
➢ If significant medical or mental health concerns exist, they must
be reasonably well-controlled.

The Procedure

❖ The procedure is done on an outpatient basis and is performed under
general anesthesia.
❖ Incisions depend on the size and contour of the breast/chest and are
slightly conspicuous. They often can be hidden around the areola or in
the lower breast crease. They tend to heal well.
❖ With FTM surgery, a mastectomy is performed to remove the breast
tissue from the chest. For certain patients, excess skin will need to be
removed as well. The remaining skin will be redraped to create a
masculine shape. Liposuction is often used to further contour the chest.
The nipple and the areola can/will be reduced and/or repositioned on
the chest after the mastectomy is performed. The nipple and areola
tend to be smaller and lie slightly more lateral and inferior on the male
chest.
❖ With MTF surgery, there are multiple decisions to be made. Breast size,
incision location, implant orientation (over or under the muscle) and
breast implant composition (saline or silicone). These will be
determined during your consultation with Dr. Chao. The purpose of
these decisions is to create a customized and more feminine shape to
the breast.

Recovery & Results

❖ This procedure is performed as an outpatient. Patients go home the
same day.
❖ You will be given specific written instructions for post operative
recovery depending on your surgery type.
❖ Patients typically experience minimal bleeding, swelling, and bruising.
❖ Generally no drains are required.
❖ You can shower the day after surgery (unless you undergo free nipple
grafting in FTM surgery).
❖ For FTM, Dr. Chao advises patients to wear a compression garment for a
minimum of 6 weeks. This is often accomplished with an Ace Wrap or a
custom garment.
❖ For MTF, you leave the hospital/surgery center in a soft cotton, non
underwire bra. This is worn for the first week.
❖ Some patients are able to return to work in as little as one week. Dr.
Chao gives you a more detailed timeline based on your specific procedure.
